College student dependents of Local 30 members are eligible for continuing dental coverage. In order to maintain the coverage, a  "Student Status" form must be faxed from the Registrar's Office of the academic institution after classes have begun. It should be faxed to the Dental & Vision Plan office at (617) 354-3315. The form needs to be faxed each semester. Coverage in summer months is continued from the spring semester.
